Instructions: The attached form is designed to serve as a summative instrument of your experience which is built on your weekly PRACTICUM LOG. (15% of your course grade)
- The first section of each part of the Checklist deals with the student's disposition (e.g. 1.5.1, 1.5.2, etc.) I will fill that information in based on my observations from your PRACTICUM LOGs, your weekly reflections and responses to postings, as well as our Skype discussions.
- For all of the other standards, you will complete the sections from your PRACTICUM LOG. By using your log as the basis for your summative project, you will get a picture of how the individual weekly activities build to your semester experience.
- The PRACTICUM EVALUATION CHECKLIST is due completed at the end of the semester.
